Instructions for use in Youth Divisions:
This infraction must be immediately telephoned to the Association Referee in Chief or the League Commissioner. It is the duty of the Referee in Chief to assist you in completing this form (should you require it) in order to submit this report to the League Commissioner within 24 hours of the game.
Infractions of a serious nature, including all incidents of Abuse of Officials or Abuse of opposing players or coaches, must be reported to the BCLOA Vice-Chair Men's Field.
